Once the RMRP pathogenic variants have been identified in an affected family member, carrier testing for at-risk relatives and molecular genetic prenatal and preimplantation genetic testing for CHH-AD spectrum disorders are possible. Diagnosis of a CHH-AD spectrum disorder is established in a proband with characteristic clinical and radiographic findings. If clinical and radiographic findings are inconclusive, identification of biallelic pathogenic variants in RMRP by molecular genetic testing can confirm the diagnosis and allow for family studies. Symptoms may include short-limbed dwarfism due to skeletal dysplasia, variable level of immunodeficiency, and predisposition to cancer.
Mäkitie et al. reported that the median adult height was 131.1 cm (range 110.7–149.0 cm) for males and 122.5 cm (range 103.7–137.4 cm) for females [18]. The radius and ulna are commonly shorter than the humerus and the femur shorter than the tibia, with the tibia being more severely involved than the fibula. Patients with CHH are classically short at birth and the shortening may increase in the first 2 years of life.
A single case report demonstrated the efficacy of an mTOR inhibitor in the treatment of anemia in an individual with CHH [Del Borrello et al 2022]. The physical characteristics of cartilage hair hypoplasia include a short limbed form of disproportionate short stature with fine, sparse hair. People with cartilage-hair hypoplasia have unusually short limbs and short stature from birth. They typically have malformations in the cartilage near the ends of the long bones in the arms and legs (metaphyseal chondrodysplasia), which then affects development of the bone itself. Most people with cartilage-hair hypoplasia are unusually flexible in some joints, but they may have difficulty extending their elbows fully.
It is caused by mutations in the ribonuclease mitochondrial RNA-processing (RMRP) gene. Extended follow up of persons with CHH revealed that about 11% of the cohort (14/123) followed for 39 years developed malignancies [Taskinen et al 2008]. A Kaplan-Meier estimate gave a probability of a cancer event (excluding basal cell carcinoma) of 41% by age 65 years. Of the 14 who developed malignancies, nine have died; median time to death was three months after malignancy diagnosis.

We studied patients from 44 different families, representing nationalities from Europe, North and South America, the Near East, Australia, and China (Table 1). Patients from 13 families were homozygous for the G mutation at nucleotide 70, whereas patients from 15 families were heterozygous for this mutation and represented one of the numerous rare mutations in the pairing allele (Table 1). Patients from five of these families were homozygous for their private mutation, suggesting parental consanguinity (Table 1). In addition to the major mutation, the base substitution G at nucleotide 211 was the only mutation found both among the Finnish (one family) and non-Finnish patients (two families). Like the Finnish mutations, most of the rare mutations in the foreign samples were base substitutions in the transcribed region.
The differential diagnosis should include other forms of short-limb dwarfism. It is interesting that over two-thirds of the patients in McKusick’s study derive ancestry from one John Miller who was married to Catherine Hochstetler. Over 70 % of the patients trace their ancestry to Jacob Hochstetler, a relative of Catherine. Eighty percent of parents of the 50 affected patients studied by McKusick were descended from either Jacob or Catherine Hochstetler, who immigrated to the United States in the mid-1700s.
A total of 13 of 14 patients who received myeloablative conditioning, and 2 of 3 who received reduced-intensity conditioning engrafted. Children who get one gene from one parent don’t have symptoms, but they can pass the changed gene to their children. A recurrence risk of 25% justifies prenatal diagnosis, which is possible through molecular analysis if the causative mutation has already been identified in a proband.

Associated Data
In patients in whom whole blood chimerism was performed, donor chimerism was 100%; in those patients with cell line–specific chimerism, CD3 chimerism was 90% donor or more and myeloid cell-lineage donor chimerism was 70% or more (Table 1). The improvement in the B-lymphocyte and NK-cell counts was not statistically significant. Four patients showed a remaining isolated IgA deficiency (pt 2, pt 12, and pt 14), and 1 patient with no successful donor immune reconstitution needed continued intravenous immunoglobulin substitution until death 4 years post-HSCT.
Diagnosis
The birthplaces of the great-grandparents of 107 CHH patients were reported2 to reside relatively evenly both in the early and late settlement regions of Finland (Figure 2A). We separately collected the information about the great-grandparents of the patients with the minor mutations and plotted their birthplaces on the map of Finland. The distribution of the birthplaces of the grandparents of the blood donors carrying the A70G mutation in the early and late settlement regions is shown in Figure 2C. Various palliative bone reconstruction procedures have been performed in patients with other short-limb dwarfism disorders. These can also be performed in patients with cartilage-hair hypoplasia. However, the risk of infection in these patients is increased, and extra attention to preventing and treating infections is necessary.
Evaluations Following Initial Diagnosis
It is inherited in an autosomal recessive pattern, which means individuals must carry recessive two copies of the gene in order to express the disorder. McKusick presented evidence that this disorder was inherited as an autosomal recessive with a frequency of 1–2 per thousand live births in the Old Order Amish [2]. Cartilage-hair hypoplasia (CHH) is a skeletal dysplasia sometimes also referred to as immunodeficiency with short-limbed dwarfism.

The essential diagnostic and demographic data were obtained through clinical descriptions, patients' photographs, laboratory results, and copies of the skeletal radiographs of most patients. Clinical Interests
We offer complete care for children with cartilage hair hypoplasia at Nemours Children’s Hospital, Delaware (Wilmington, Del.) and select Nemours locations. The immunodeficiency in cartilage-hair hypoplasia may be an isolated T-cell immunodeficiency, isolated B-cell immunodeficiency, or combined T-cell and B-cell immunodeficiency. Occasionally small insertions, duplications, or triplications in the promoter region increase the distance between regulatory elements (e.g., the TATA box and the transcription start site). An increase of bps between the regulatory elements leads to promoter inefficiency and reduced RMRP transcript levels [Ridanpää et al 2001, Nakashima et al 2007]. Such variants have been observed in compound heterozygosity with pathogenic variants within the transcript.
Genetically Related (Allelic) Disorders
Single individuals with biallelic RMRP pathogenic variants and immunodeficiency without skeletal features [Ip et al 2015] and with Omenn syndrome [Roifman et al 2006] and normal childhood growth [Klemetti et al 2017] have been described. Since the mutation G at nucleotide 70 was common in several countries, we asked the question whether this mutation had a single origin or if this nucleotide was a hot spot for mutations. Eight homozygous and nine heterozygous patients and their family members, and two Amish CEPH (Centre d'Etude Polymorphisme Humain) controls were genotyped and haplotypes were reconstructed. Chromosomes from different nationalities and the Old Order Amish carrying G at the nucleotide 70 shared the same alleles with the Finnish major haplotype (Figure 1B). Twenty-three of the 27 haplotypes were similar, at least in a region of 60 kb around the RMRP mutation, clearly pointing to a common origin of this base substitution rather than due to frequent occurrence.
The samples were collected in eight regional centres, and the sampling density is representative of the Finnish population at the start of 1900. A sample consisted of the buffy-coat of the whole donation (about 450 ml) generated as a side product in the routine preparation of a leukocyte depleted red cell unit. DNA was extracted using a modified salting out protocol.18 The ethical committee of the Finnish Red Cross Blood Transfusion Service approved the sample registry. Cartilage hair hypoplasia is quite rare, occurring in only 1 out of 20,000 births. It is significantly more common in certain Amish populations, occurring as high as 1 in 1,300 births. Cartilage hair hypoplasia is a genetic disorder linked to mutations in the RMRP gene.
Cartilage hair hypoplasia is a genetic disorder that affects the metaphyseal area of the long bone — the wider part at the end — causing lower-extremity abnormalities. The condition is an autosomal recessive disorder, meaning it is passed on to a child by both parents. The defect is caused by a problem with the spherical or rod-shaped parts of a cell called mitochondria. On the other hand, the mitochondrial localisation signal region has been reported to reside between nucleotides 118–175 in the mouse29 which corresponds to nucleotides 118–167 in the human RMRP. This stretch harbors four different substitution mutations; two patients were even homozygous for this kind of mutation. Functional studies are needed to further clarify the viability of mutations in the nucleolar localisation signal region and the significance of the mutations in the mitochondrial localisation signal region.
There is marked shortening of the metacarpals, metatarsals, and phalanges, with metaphyseal cupping and “cone-shaped” epiphyses. Costochondral junctions have reflected mild flaring of the lower ribcage, slightly anterior angulated sternum, and lumbar lordosis. There is commonly delay in appearance of the proximal femoral epiphyses but absence of the proximal femoral metaphyseal changes that are typical at the knee.
Micromelia may be detected early in pregnancy during ultrasound follow up, but is not specific. Cartilage-hair hypoplasia is a disease affecting the bone metaphyses causing small stature from birth. Skeletal dysplasia significantly impairs the normal activity of these patients. Care directed by orthopedists and physical therapists is necessary to monitor and treat these limitations. Likewise, even in the face of excessive laxity of the joints of the hand and concomitant shortness, we did not encounter any clinically significant complaints. Inasmuch as a large number of patients were Old Order Amish (65 patients), the usual types of employment probably correlate with the lack of complaints (blacksmith, farmer, lumber work, rough and finish carpenter).

Although he was successful, Wilson still berated House for his drastic actions, telling him he could have been killed, regardless though aside from a few slight neurological differences House was completely fine and Amber free.

Eventually House managed to convince Cuddy to prescribe him with sleeping pills, confessing to her that he hadn't been able to sleep since Dr. Kutner killed himself. House actually enjoyed her company in this time up until she began growing more sinister, eventually culminating when House learns that by hiring Karamel she was actually trying to kill Chase, due to his strawberry allergy and Karamel's strawberry body butter. House talks to Wilson again, telling him what's wrong with his patient, then confessing he still thinks he's ill. House goes to his patient and apologizes for making severe mistakes while treating her, all the while Amber is impressed with him. He didn't feel anything from the apology however, once again telling Wilson he feared he had Multiple Sclerosis. House realizes the danger of her and by extension his own subconscious by this point and in spite of her trying to convince him that she was of benefit to him, House tried to sleep to get rid of her, but she persevered and kept trying to tell him to return to the hospital.

House probably should have known that once he was suffering from hallucinations, he should have sought out outside help instead of trying to self-diagnose. Treatment of hallucinations as a symptom is dependent on the underlying condition causing the hallucinations, and some treatments can actually be counter-productive. With each failed method, the Amber hallucinations became more sinister, cruel, and violent, even going so far as to calmly slice her own arm open with a scalpel in Under My Skin. House resorts to an Insulin shock and it temporarily works, until House sees Amber return at a restaurant singing. As a hallucination she had mostly the same personality however was slightly more sinister and incredibly kooky and eccentric, but as with her living self she eventually grew much kinder and more sympathetic towards House. House had no choice but to detox from Vicodin, telling Cuddy everything, before allowing her to search for and get rid of all of his secret stashes, in spite of Amber's protests and strong disapproval.

As a subconscious of House, House was terrified of it as the hallucination had been manifesting intents to harm others as well as meticulous homicidal tendencies. Slowly she started influencing House more and more, starting by convincing him to hire a specific stripper for Dr. Chase's bachelor party named Karamel, then convincing him to have Dr. Chase put an implant into the deaf patient's brain which allowed him to hear again. This backfired however as Dr. Cuddy decided to Foreman in charge of the case as a consequence to this. The following patient was a man with muscular dystrophy, and in order to weed out more competition House elected to split the remaining candidates by gender, Amber however requested to go onto the men's team, which House was somewhat indifferent to although realized why she asked this. Amber figuring all of the male applicants were incompetent decided that she would go on their side, solve the case and therefore eliminate all of the women (AKA the genuine competition in her mind). This plan somewhat backfired however when the women began succeeding more and more in the case while all of the men and Amber were nearly fired due to all of them simply running tests in the lab and never bothering to actually see the patient.
She quickly mutinied and took eight others, with only Cole staying behind to do the work. However, she returned fifteen minutes later to rejoin Cole and took the car to a car wash. She aggressively made suggestions and even broke the rules to keep medical records on the patient on her PDA for easy referral. Lawrence Kutner ratted her out for it but almost got fired for "squealing". In the end, her willingness to compete and take risks made sure she made it to the final ten.
The team confirms House's diagnosis of Amber, but there is nothing they can do to treat her. Wilson weans Amber off anesthesia in order to spend her last moments alive with him. The team comes in one by one to say goodbye to Amber, and after Wilson himself says goodbye, he shuts off Amber's bypass and she dies peacefully in Wilson's arms.
